Your modules.conf looks ok to me...
I'm really not sure what else to tell you.
The only thing that comes to mind is to try backing the volume down from
the maximum and see if it changes anything. You've probably already
tried that.
Does anyone else have any ideas? my meager alsa knowledge goes no
further.
