Some 2.6.x kernels had problems with USB unloading.  Try 2.6.5.

rmmod snd-usb-audio

The midiC0D0 file is for the MIDI port of your first sound card.

There should be a bunch of messages from the hotplug scripts.  Please
make sure that hotplugging is enabled.

What are the contents of /proc/bus/usb/devices?
