What version of Amanda?

Did you run this in the background (without the -f flag)? Do you have
an amflush.NN file in the amanda.conf directory? What's in it?
If you used -f, it would have put the same information to your screen.
If you still have that, or get it again, that might be useful.
Did you run amcheck before amflush? You might want to do that, too.
Any chance the tape is write protected?
