Perhaps I missed it since I'm jumping in mid-thread, but are you using real tapes or vtapes (using the FILE driver)? If it is real tapes, check your changer to verify that there is a tape in the drive and your changer can move it properly (using mtx or whatever tools you have available) and check your system logs for more descriptive SCSI errors.
