We run SA and anti-virus in pre-queue mode. We use Postfix' smtpd_proxy_filter and Postfix' Sendmail milter API. smtpd_proxy_filter is more stable. Sometimes the milter on Postfix bails out. I haven't had time to investigate that any closer yet.
