Well, it reports on the values of time-taken that are listed in your
log files. My guess, is that your web server is reporting lots of
requests as '0' time taken. Perhaps those are error or 'not-changed'
responses (which essentially have no contact -- usually listed as o
bytes).

As for what portion of the request this tracks, again, that's up to
the web server, but most likely it between the time it receives the
request until it send the header. You should check into your web
server documentation (or source if available!) for more details.
