No, you need to publish just once for Android Market. You can choose to not publish to some countries, but that's all you can choose. Some carriers may have their own stores, but I used none.
As for portability, if you develop with the SDK and without making any assumptions regarding the hardware (screen size for example), your app should run on any device.
