Ok...this may be coming out of "left field", but it's just a thought. Take a look at that ASSIGNEE GROUP IDs for the companies that are having this strange issue.
Somehow or another, we had several of our companies assigned to the same GROUP ID. It was truly weird and caused some of the same strangeness that you're describing below.
