My one suggestion seeing that it comes and goes, is to check any proxies that 
might be between the client and the web-server.

If you have proxies configured in IE you can simply disable them. Dependant 
upon which version of IE you might need to ensure that ALL IE windows are 
closed to pick up the setting change...

A long time ago, we had to code rules into the proxy servers to BYPASS the 
Remedy Web Servers in order to correct "strange intermittent issues"...

I wonder then if its any regional settings or maybe even the time settings on 
her local client is having some sort of a conflict with the login process? I 
think it may be safe to assume that the locale is checked for during logon 
time, so if there is something wrong with the locale settings, it may be 
causing the authentication to fail?? Though I can't really tell what that might 
be?
 
Check her Regional / Timezone / Local Time settings..
 
All I can say is it seems strange..

And it's back.  My customer is not able to logon to the Mid Tier.  I'm able to 
log on as her from my computer.
 
I noticed on the Manage User Licenses form that her Connect Time is normal but 
her Last Access Time is 12/31/1969.  When I log in with her credentials from my 
PC the Last Access Time is normal.
 
I've logged an issue with BMC, anyone see this before?

We don't use LDAP to log into the Mid Tier.  We just create our own user id's 
and password using the AR System User Profile form.

I able to log onto the Mid Tier ("user tool").  I'm also able to log in as 
another user just fine.  But the user keeps getting Authentication Failed when 
logging onto the Mid Tier from her computer.
 
I had her flush out her history in IE, Delete her Temp Files in IE and Delete 
her cookies in IE.   I had her flush her DNS.  I also flushed the Cache on the 
mid tier server.  Still no luck.  
 
We are using Mid Tier 7.0.1 patch 3 with AR Server 7.0.1 on a Windows 2003 
server.
