My one suggestion seeing that it comes and goes, is to check any proxies that might be between the client and the web-server.
If you have proxies configured in IE you can simply disable them. Dependant upon which version of IE you might need to ensure that ALL IE windows are closed to pick up the setting change... A long time ago, we had to code rules into the proxy servers to BYPASS the Remedy Web Servers in order to correct "strange intermittent issues"... I'm also able to log in as another user just fine. But the user keeps getting Authentication Failed when logging onto the Mid Tier from her computer.
