Try disabling the workflow on your Init-Form that opens the form and use the Home-Page: ar server setting instead. Multiple clicks to the home button will (should?) not reopen the form -- at least that's how it works in the user tool. I'm guessing that since your form wasn't opened through the Home Page functionality initially, clicking the button opens another instance of that form.

I don?t want to have to add a Logout button to each of my forms, so can I add a Logout button to the web toolbar?
