We use Remedy to manage the metadata for a 13M LoC mechatronic application using ~50 custom forms. Besides the bug tracking application we started with 10 years ago we manage Goals and Product Backlog Items, Build Schedules (some nice connections under that to perl and ruby scripts that build code), Organizations (used to keep track of who's responsible for what), Licensing and Time Clock to mention just a few.
