I've not dealt with this specific hotfix, but we have found many odd caching behaviors with 7.6.04 that have lead to us thinking fixes didn't work when they really did. Often changes are not sync'd with a flush of the midtier cache and sometimes not even with a restart of the webserver. We've had a couple of changes that wouldn't apply for up to 3 days despite all our efforts, then suddenly appeared as some component finally reached. I've worked with Remedy for 8 years and never seen the odd caching behavior of this version.
So I would recommend you first ensure you've done the following to rule out a simple cached prior version of the form/workflow: 1. Flush Midtier Cache 2. Restart Webserver (if you have the luxury) 3.
