I liked the article - because it challenges a common accepted idea
"root-cause" -- there is never "one" root cause -- there are many.

Fire = heat, fuel, oxygen (take away one -- you don't get the result)

The Titanic sank -- why?

Rudder too small
Lookout people not properly trained
Not double skinned walls
Can't rivet on curved metal
Hit ice-berg
Went too fast
Travel at night

Any one of those could have "prevented" the issue.



Root-cause should be relabeled "Contributing factors"  (Or something like
that).
(and it should be a list :) (or a table in Remedy talk))
