My solution to this (only tried last week hence no post yet) was to
disable on-access scanning of the ClamAV directory by the onboard AV. As
we are a small outfit and the mail server has multiple roles there is
24x7 scanning of all files written to disk. Since I excluded the ClamAV
directory from scanning I have had no alerts for high CPU usage.

Your situation may be different so your mileage may vary.
