The general sentiment, I noticed, is that once an IP (range) is blocked it stays blocked. At least with manually maintained lists (we all have our own list no? ;-) but afaik also with a number of stricter block lists. And you unblock on the basis of false positive reports or something. It's a pretty sure bet that once and IP (range) has been tainted by sending spam it's unlikely to ever become "clean". In those rare cases it does you can unblock on a case by case basis. By hand.
