and now it's a disaster. The extension which does moh still works but I
can no longer hear anything during other calls. The CLI monitor shows the
calls coming in and that the gsm's are playing but I don't hear anything

Outgoing calls are working but I do see thess messages on the CLI when I
make an outgoing call:

These messages repeat several times but the outgoing call at least works.
That's annoying - I don't (think I) see the problem here with FreeBSD 5.4 (I
can certainly run the "demo" context from the default asterisk configuration
without any problems, and incoming IAX->outgoing SIP seems to be fine).
The warnings above would suggest a problem with codec
recognition/translation, though looking at the code around those lines, the
problem has already occurred before those functions are called.
Could you try the original patch I submitted for 1.2.12, at
http://www.freebsd.org/cgi/query-pr.cgi?pr=ports/103426 (NB patch is against
1.2.9.1) to see whether the regression occurs between 1.2.9.1 and 1.2.12 or
between 1.2.12 and 1.2.12.1?
