I use the same strategy and it works just fine, however, I did have an
issue, here's the scenario:

SIP Ext 1 +---+
              |--- NAT --> Internet --> NAT --> Asterisk
SIP Ext 2 +---+

Each of  the SIP extensions work individually, but if I try to use both
of them, only the first one registers.

Just a thought: Try using a different SIP port for one of the extensions, if possible.
