I've never run against a commercial PBX that didn't need maintenance.  VM hard 
drives fail, there are new extensions needing to be added/modifed, people 
forget their passwords, trunk cards fail...  Asterisk is every bit as stable 
as the old-gen KSUs and PBXs.
