I guess its a question of how many resources you want to give to MOH.
Even the sound card requires an IRQ and A to D conversion etc.
I think the worst thing would be to use the CD player as the source
I found that the streaming audio MP3 24K bits (mono) was a good
compromise between quality
selection and resources. Anyway, any system that has to put people on
hold for any lenght of time is
going to upset the caller no-matter what music they hear. I
personally would like to hear old radio broadcasts
like Burns and Allen or better still Monty Python's the "parrot"
sketch for MOH.
As a good idea of what not to do check out Rogers Cable, my blood
pressure goes up just thinking about it!

You mean like app_ices? The native res_mp3 should be able to do it too, I
think. You could even use something like lastfmproxy to get something more
dynamic, although last.fm seems to be kind of flakey for me.
Personally I just prefer to take a $10 radio and plug it into the sound card.
Bandwidth is for pr0n^Wresearch, not for providing MOH. :-)
