It all depends how you setup your features in you extensions.conf, for isntance you can have something that stuffs in things to the realtime db or you can have some uber scripts writing to your own db which is read and writen upon feature execution. I know if you made you own functions that dealt with the later technique you wouldnt have to worry about restart asterisk and loosing your forwardings etc... there is a nifty module for doing mysql things in asterisk, its in the asterisk-addons.
