I only have one Trixbox server running, the rest are various other installs of Asterisk.

The one I am running is Trixbox 2.0.0

Here is my 'top' :
top - 11:52:14 up 276 days, 17:46,  1 user,  load average: 0.00, 0.00, 0.00

As you can see, it has been up for 276 days. It doesn't display any of the symptoms you have described. Now since the initial install, have you installed any new packages?

I've not updated any of my packages in a few months.

Go to your Tools/Module Admin, one of your modules may be causing the issues your talking about. You may have to search on each one and it's version to see if there are any bug reports on it. To get you started below is the listing of the ones I have installed, and all seems just fine here.
