I'm quite happy with my iPhone running SIAX. There are also a few strictly SIP clients, but I suspect IAX might work for you too, and it avoids the firewall issues.
I will say, you need a fairly decent wifi connection for it to work well. I had intermittent problems at a campground with voice calls over wifi. It may have been oversubscribed. It's worked well for me when it was an access point that I had control over (read that as : an access point typically seeing very little use, so bandwidth wasn't an issue.)
