Thanks for the answer jlc. The router runs the DD-WRT VOIP version, which include Milkfish. And Milkfish is a SIP/RTP proxy that fixes NAT problems, based on SER. No portforwarding should be needed(and there is no forward to the Fritz!box and the Sipura SPA2000).
In order for it to work, SIP devices behind it needs to register both with the IP-phone provider(s), but also with Milhfish.
