Ok, all working now. The situation was complicated by the fact that my test environment has no telco BRIs. Our existing old Avaya system is plugged into a PRI, and has a So8 module which provides BRI sockets. I hadn't properly configured this module, and I also had an Asterisk dialplan error. Fixing these seems to have sorted things out.
