I have had this happen. NM is asking the card to rescan after resume, but the card is hung for some reason ("can't reset hardware" is a giveaway). As for debugging it, you can do what I hope to do when I next have some free time: stuff ath5k_hw_reset with printks to get an idea of what is working and possibly narrow down which sequence gets it stuck. BTW does a subsequent suspend-resume cycle help at all (instead of reboot)?
