Traditionally, health items are used as soon as you pick them up, but some games like golden axe have given you a small inventory to manage. I think that walking right and mindlessly mashing buttons could get boring fast, so as much variety to keep things interesting would be interesting. This might include intelligent AI, challenging bosses with patterns to learn, multiple characters with different backstories and/or strengths and weaknesses, if this is fantasy themed then maybe an occasional mount such as a horse or a dragon, a scoring system and combo counter, (killing enemies, racking up combos and picking up useless miscellaneous items would contribute to the score), a variety of melee weapons with different range and durability, several game modes and unlockables to extend replay value, and if possible, cooperative play.
