In terms of fully playable, either with a screen reader, OCR and/or audio cues:
Sequence Storm: A rhythm/racer where you time notes in a Simon-like fashion to increasingly difficult music complete with DLC and a level editor. Uses SAPO by default, but NVDASAPI works fine.
Code 7: A futuristic hacking text adventure where you as the main character has to stop the virus Code 7, against an evil AI. Includes sounds, music, voice acted dialogue, and truly amazing story.
Audio Wizards: I'd be half amazed if you didn't know this already!
Alt Frequency: You know what? This one's hard to explain.
Skullgirls: A 2D sidescroller beat-em-up tagteam game, complete with much character DLC, story mode, multiplayer, comprehensive training and tutorials for each character, fully voice-acted dialogue. Uses the Tolk libraries for screen reader output, and a wide stereo field.
A Blind Legend: Again, surely you've heard of this one. It's on Steam, better than ever.
