Thank you for bringing NVDA-reading issue to our attention. We have changed the format slightly, so as not to use column-based (horizontal) checkboxes. If we have only made it worse in this change, please let us know!
The questionnare is completely anonymous, but I will make it so the age field isn't mandatory. An age-range can be filled in instead of an exact age, if this is a preference.
