It is worth remembering that there are no "new" types of accidents and lessons previously learned remain relevant today. We just need to keep reinforcing those vital aspects that can reduce the risk of accidents.

However I do agree with Robert that the GFA hierarchy appears to be so lax in the investigating and the reporting of accidents. With yesterday's fatality at Gympie, there have now been three fatal glider crashes in about 12 months here in Australia. During that time there have also been a number of accidents involving serious injury.
I know we all extend our sympathy to the friends and families of those involved.
The problem is that we are currently incapable of learning from these terrible happenings. This is not because we, as individual pilots and instructors do not wish to learn from them, but that, even where investigations into accidents do occur, the GFA does not publish any accident reports.
We are therefore incapable of learning where our system failed in these instances and thus remain at risk of becoming the victim of the same 'system failure'.
A perusal of the GFA council/board minutes will show you that this issue has been on the GFA's plate for some considerable amount of time, with no apparent progress.
I would ask that individuals speak to their state representatives, requesting the council/board to instruct the executive to expedite this issue and, in the meantime, report in detail on what is holding up the issue. Such a detailed report will allow the accumulated wisdom of the membership to be applied to help the executive find a satisfactory solution with as little further delay as is practicable.
I am sure none of us want to see anyone killed or seriously injured due to the lack of adequate accident investigation and reporting, but that is exactly what the current lack of progress on this issue is inviting.
