Shelagh,
Thank you for the very good review/edit of this chapter. I have made some more changes, partly based on your comments in this note and in the file, and have uploaded the revised chapter here:

2. I wondered as I was reading, is there a particular reason that the
order of the sections is that way. My preference would be to put the
order of the instructions in the same relative order as the sections in
the dialogs.

Good point, and good catch. I have changed the sequence.

In heading and footer appearance would it be hard to have a reference to
the section in the formatting chapter explaining how to use the dialog
in Figure 9?

I added a reference to Chapter 10 (Using Styles in Calc), which is where this information should be. However, the explanation is in fact missing from Chapter 10, so I've made a note in my copy of that chapter to add it.

Figure 10 (icons) plus the icons in the text showing the custom header
buttons are different to what I am seeing. Is this a change in
oo-dev-2.3 or because of the OS I'm using?

Probably it's the OS, but you should be able to change this. Go to Tools > Options > OpenOffice.org > View. Look under Icon size and style. The second box probably shows "Automatic". Change that to "Default" and you should see the same icons as are in the text and graphic.
