Well, the problem is solved.
 
I did not check the obvious: I had 2 identical (blank) records in table InventItemLocation. Axapta expects only one instance with identical warehouse, so this caused the confusion. After having deleted the record using Table Browser, everything looks fine again.
 
How the duplicated record ended up in InventItemLocation ? Well, probably during data import in the week-end.

I am confused here.
 
1.Firstly,  So you have ONLY one record in InventTable  Table, but you see two records in InventTable form?.
   did u actually search on the form also?. or just you see twice the Item Number displayed.
  ( I think you might have already done )
 
2. Secondly,  Did anyone of ur developers make any chenges to InventTable ? ( Form/Table )
 
3. Thirdly, Did you also  try to create already existing Item to see if it getting allowed ( in our system )?.( I think you might have already done )
 
Its pretty interesting and scaring to me.
 
 
* We do have a plan of scheduled compilation of full application on the on going basis.
* We do Check/Syncronize if we have strange problems
* We do re indexing on a table ( when no one is there )
 
Can you please help me by replying those.
