I'd recommend to try the mtx-changer script alone first. I suspect there is a timing problem in there. My experience is that it's usually quite useful to use the wait_for_drive function instead of a pre-determined timeout, because - especially with DLT - the time it takes for the drive to accept a tape can greatly vary, and with uninitialized tapes it can take very long.
