That looks like a correct RFC-2184 encoding of MIME parameter values.

Normally it should be different, like this:

Content-Type: application/vnd.ms-excel;
name="=?windows-1251?B?5+Dv8O7xIO7yIDAyLjAzLjIwMDko8err4OQpLnhscw==?="
Content-Disposition: attachment;
filename="=?windows-1251?B?5+Dv8O7xIO7yIDAyLjAzLjIwMDko8err4OQpLnhscw==?="

That would seem to violate RFC 2047:

+ An 'encoded-word' MUST NOT appear within a 'quoted-string'.
