Yes, I have! Quite frequently as of a couple of weeks ago but it seemed to have gone last time I updated the version (about a week ago) and did a full rescan.
It was quite unusual, even the web interface was one track behind, yet SS knew what track was playing because iTunesUpdate/SlimScrobbler were catching the correct track. Stopping and restarting the playlist was fixing the problem each time.
