In my opinion, it's pretty usable at this point. There are still bugs though.

Also, assuming you're running the SVN trunk, do you need to provide
your own MySQL back-end. And if so, can that be 5.1 or does it still
need to be 4.x?

You do not need to provide your own MySQL database. Even if you are on svn,
SlimServer will launch (and stop) the built in copy of mysqld.

If you wish to run your own copy, it's been tested on 4.1 and 5.0 (which is
what ships). I've not tested on 5.1, but I don't see why it wouldn't work.
The only known problems are with 4.0.x, which doesn't support "SET NAMES" for
UTF-8 support.
