What OS? Can SqueezeCenter write to your playlist dir? Are you rescanning your library after the updates?
In general, the plugins and other features that write an OPML try to write it to your playlist dir. Some of these, but not all (I'm not sure about favorites) will fallback to writing to the cachedir if the playlist dir isn't writable. In some cases (all?) if the OPML file can't be written, the change is still made to the database.
