The problem goes down to extrude keymapping.

The "main" extrude operator now uses the Extrude Along Normal operator, for 
which this is the correct behaviour.

Back when the extrude operators where recoded for 2.5x there were separate 
hotkeys for "extrude region and move" (free of constraint) and "extrude region 
along normals" (which is currently mapped to E).

People can already map E to either operator. We can certainly change the 
default keymap to either, but I think it's pointless to try to do a smart 
extrude operator that does an hybrid version of both depending on arcane 
conditions, as this will only frustrate people.
