That patch dates back to when I was using gcc4 but the book was still
at gcc3.x.x. I used all patches then as there was a place where folks
building with gcc4 could find fixes and I stuck everything there until
the book went to gcc4. Cyrus-SASL hasn't had an upgrade since then, so
it hasn't been touched, I believe.
