In theory all muds should be accessible, but the amount of text you have to process at once can make it difficult to keep track of what's happening.

Consider a battle scene where you're facing say three other enemies, and each of you can take an action every second or so. Your screen reader can theoretically read all this information, but by the time it finishes, you'll probably be unable to react.

Those muds that are considered more accessible have usually taken some steps to make themselves friendly to screen readers, whether that be minimizing the amount of text you have to process, or making it possible for you to play with sounds as well as text. The most obvious example of this is Alter Aeon, where almost everything has a sound effect associated with it.
