That is a combination router and modem. There is a place for a normal phone line to go in the back, should be on one end. The other ports are for Ethernet cables to plug in.
They probably also included a wireless card which is supposed to be installed in a laptop, they assume you own one. If you already have a wireless connection, you can leave it in the box. What you will need to do is plug in the power adapter first to the wall outlet then to the back of the router. They should also have included a CD for installation. Run that and it should be able to walk you through the setup. One of the lines included is to connect the modem to the phone box. The second, thicker and larger ends is the Ethernet cable and will connect from the port on the back of your computer to any of the 4 ports on the back of the modem. There is a series of numbers on the bottom of the router as well. One of them will be the password to make changes to the router online. I think they all use the same web address which is 192.168.1.254 and then you will need someone to read you the numbers off the bottom of the router to move on. You can change the password to something you remember so you don't have to worry about the numbers later on.
