Start from the instructions here:

Make sure the BrailleNote is using the default computer name (delete "Flash 
Disk/Dictionaries/DevName.sys" if it isn't, then reset).  The code for the 
BrailleNote is always 0000 regardless of how you may otherwise pair.  Put 
BrailleNote into terminal mode with Bluetooth as the port, then on the iPhone, 
go to Settings > General > Accessibility > VoiceOver > Braille and turn it to 
on.  If Bluetooth isn't on, it asks you to turn it on.  Then it scans, and 
should find your Apex, for which you pair 0000.

Yes, it's fiddly and annoying, really.
