    [doc,man]: Correct and update hyphenation stuff.
    
    * doc/groff.texi.in (Manipulating Hyphenation):
    * man/groff.7.man (Request short reference): Fix error that crept into
      groff 1.23.0 documentation; hypenation exception words have no
      association with formatter environments (though that is a contemplated
      change; see Savannah #66387).

    the report.  Problem introduced by me in commit 493a01c02d4, 28 June
    2020.
    
    Also:
    * Document unportability of `\%` leading a word for hyphenation
      suppression.
    * Reform terminology: (hyphenation) exceptions -> "hyphenation exception
      words".
    * Mention indexed characters alongside ordinary and special ones.
    * Kick disclosure of formatter warnings into footnotes.
    * Favor active voice over passive.
    * Recast.
    * Parallelize input line breaks between Texinfo and man(7) documents.
    * Update annotations for future directions.
