When working with text that is quoted, smartquotes automatically replaces the
inch mark with \[lq] and \[rq] (as you know).

However, this would appear to be a simple toggle on/off, such that if there is
a footnote, which itself contains quotes, inside another quoted text, the
footnote's quotations are reversed.

Perhaps this is a non-trivial fix, although perhaps smart quotes could start a
new right/left toggle when entering footnotes (probably end notes as well)
since they are displaced from the rest of the text.
