You could do this by using @defindex right now.

Seems reasonable to me. Perhaps the links/bookmarks (you're talking pdf
I assume, but it would also be useful in html and even info) could
always be there, actually. But a command to control the merging
algorithm is probably desirable. Maybe as a percentage of the total
number of entries.
