The entry that ends with ``(suexec)'' that I described in my previous
comment occurs anytime there is a server error (500).  The message
displayed in the web browser is always  "Premature end of script headers"
but httpd-suexec.log usually has something like "file has no execute permission"
or "directory is writable by others".

Here's a process table example of these ``(suexec)'' entries:

You may also consider fixing this so that it reports the name of the script,
and not just the word "suexec".
