I was about to post that I had added some more debugging and discovered that mod_proxy was emitting info that it had initialized worker->id 0 and worked->id 2, but not worker->id 1, and that mod_proxy_ajp was being handed worker->id 1. Dunno if that was the problem being resolved in the fix. I will try 2.2.1 right now.
