I can retrieve that file, so that suggests that the server was having
issues.  Either it was overloaded (too many requests) or was being
restarted while the client was attempting to install.

The package system will retry 4 times by default before giving up and
aborting the operation.

yes, i have tried 3 or 4 different times with build 09, and once with
build 08.  all same results.  i also had a user trying to do an onu on
an existing host with same errors

This indicates that there is likely an error in the Apache configuration or a network issue with the clients.

file for the service (i'm understanding that the service should be in a

I can't be certain of the configuration since there seems to be no shell access to the server in question. However, there are sample Apache configurations here for both the proxying and caching that you should have enabled:
