I have a very similar setup and I can confirm your problems:
I also hear distorted dial tone and get the overflow message.
Furthermore at startup, the following message appears:
Jun 29 13:01:56 WARNING[1101064512]: timer.c:112 _set_interval:
Requested a timer with 1000000 nanosecond interval, but system timer
reports a resolution of 4000250 nanosec. Timing may be unreliable!
However following things seem to work without problems:
Receiving calls via SIP
T.38 Gateway from SIP to T.30
Regarding your second problem:
In my case it depends on the VoIP provider. With sipgate.de and using
my ISDN phone to enter the number first and then pressing dial, Speech
works immediately, although there is still one overflow and underrun
message:

But using another VoIP provider (sparvoip.de), I receive a lot of
overflow messages. The first overflow block appears until the
connection is established. Then after picking up the call I cannot
hear or say anything and overflow messages appear again. Strangely,
pressing a DTMF tone stops the errors and enables sound (both speaking
and hearing):
