We plan to build some routine reports inside Care2x but it is difficult to
predict all possible reports that users might need. I know there are some
report writers, which will extract data out of databases. Does somebody have
experience of some good ones you advice to use with Mysql - free or
commercial.
A. In proprietary software (paid) software:
I know off Crystal Reports. It is the grandfather of them all (they date back from dBase and Clipper!!!). It is expensive and needs windows to really work well. I have not seen its last three editions up to ver. 10. You will be needing an in-house expert just to tame it to yours need, but after that the user's imagination is the limit.
B. In the Open Source World there some clones of the above to look at:

You can also define levels, subtotals, and a grand total for the report, merge the data into a document, generate address labels, or even generate a complete ER-diagram from your database"

"DataVision is an Open Source reporting tool similar to Crystal Reports. Reports can be designed using a drag-and-drop GUI. They may be run, viewed, and printed from the application or exported as HTML, XML, PDF, LaTeX2e, DocBook, or tab- or comma-delimited text files. The output files produced by LaTeX2e and DocBook can in turn be used to produce PDF, text, HTML, PostScript, and more."
I would bet on *Agata Report* as it has a knowledgeable base of developers.
