I asked this question myself, too. I doubt somebody will 
have the will to change this numbers. Finally, I decided
the new installer to have all possible configuration as
the old one (you know, backward compatibility). Maybe in 
the future we will have "novice" and "expert" installers.

You can send the modified Turkish ICD10 to my mail. I 
will include them in the next beta. You will help me a 
lot if you send me modified 
/installer/db/icd10/icd10_tr.csv file instead of an sql
file.
